LEVEL OF SERVICES INVENTORY-REVISED REPORT

The Level of Services-Revised (LSI-R) is a structured Risk/Need assessment meant to identify needed level of care and services for a Criminal Justice population, as well as explore 10 subcomponent areas where specific interventions or supervision rules may be needed.

The following test findings reflect professional ratings made on the basis of Mr. Sample 's responses to a semi-structured interview and a review of his available records. As with all such tests, the validity of LSI-R results is limited by the available information and Mr. Sample 's honesty and self-awareness.

This report should be taken as generalized probability statements and should never by themselves be a sole basis for placement or care decisions. Further clinical verification is needed to assist in the interpretation of test findings in light of Mr. Sample 's unique history and present circumstances.

The findings below focus on problems, deficits and pathology, and so may de-emphasize Mr. Sample's strengths. Because of this, use without collaboration, other than for the screening purposes for which they were intended, may be misleading.

This is a CONFIDENTIAL REPORT meant for qualified Mental Health, Correctional and Substance Abuse professionals. While feedback of test findings to clients is highly encouraged and should be an integral part of therapy and placement planning, inmates should not be given copies of this report, as they are likely to misunderstand report contents and their tentative nature.

OVERALL RESULTS

Mr. Sample 's overall score of 33 on the LSI-R is at the 80th percentile for a male inmate population. This score falls in the Moderate category of Risk/Needs.

Using standard cutoffs would suggest the following:

SUBCOMPONENT ANALYSIS
The LSI-R id divided into 10 content areas that can provide information useful for planning rehabilative programs and for the formation of supervision rules.

CRIMINAL HISTORY: Mr. Sample has an extensive past criminal history, which is a poor prognostic sign. He was rated as having five or more of the following historic risk factors: prior convictions, present multiple offenses, arrest before the age of 16, prior incarceration, escape history, Institutional misconduct, new charges while on probation, probation suspension/revocation, and a history of assault/violence were not found. Such a history of criminal behavior places Mr. Sample at much higher risk of both acting out and recidivism.

EDUCATION/EMPLOYMENT: A moderate level of educational and vocational adjustment problems was found. Mr. Sample may have significant difficulty finding and maintaining employment and may have done poorly in school with him having had behavioral and/or learning problems.

FINANCIAL: Severe financial difficulties and a reliance on social assistance exist.

FAMILY/MARITAL: Highly dysfunctional marital and/or family circumstances exist.

ACCOMMODATION: Mr. Sample 's living situation is a positive one.

LEISURE/RECREATION: He lacks positive prosocial activities and does not use his time constructively.

COMPANIONS: Mr. Sample social support group is extremely antisocial and is not likely to support prosocial activity.

ALCOHOL/DRUGS: Very Severe Alcohol and Drug use appear to be a factor in Mr. Sample 's criminal activity.

EMOTIONAL/PERSONAL: Severe emotional and personal problems were apparent, with his mental health problems likely to effect risk of recidivism.

ATTITUDES/ORIENTATION: Mr. Sample displays a negative attitude and evidences severe criminal thinking tendencies that may lead to recidivism and interfere with treatment.

SPECIFIC TREATMENT NEED/SERVICES RECOMMENDATIONS

Due to Mr. Sample 's extensive history of past legal involvement, extreme care must be taken in placement and monitoring. A high degree of structure and strict supervision rules with clear boundaries are indicated. He needs to be closely supervised and held accountable for his behavior.

As Mr. Sample displays significant attitude problems and is likely to have Criminal Thinking tendencies, specific treatment for this is indicated.

As a moderate level of educational and vocational adjustment problems were found on the LSI-R, he will need assistance in finding and maintaining employment. Referral to vocational rehabilitation may be necessary. Further educational attainment should be fostered. Evaluation for learning problems and special education assistance may be required.

Help resolving Mr. Sample 's existing financial problems is needed. This may include budgeting, help gaining employment and social service triage.

Marital and family psychotherapy may be needed as highly dysfunctional marital and/or family circumstances are said to exist. If these problems cannot be resolved, then limiting family contact should be considered.

He needs to develop positive prosocial activities and learn to use his time constructively. Referral to programs may be needed, as idle time may be a risk factor.

As Mr. Sample largely lacks a prosocial support group, he needs encouragement to develop positive peers. Because his present associates are unlikely to support prosocial activity and may lead him into criminal activity, contact with negative friends should be limited.

Evaluation and treatment for Substance use is needed.

Mental Health assessment and treatment is indicated.
