PERSONALITY INVENTORY FOR YOUTH REPORT
Mr. Sample was referred for psychometric screening to aid in diagnosis and treatment planning. To this end, Mr. Sample's mother completed the Personality Inventory for Youth.
The following test results are without benefit of clinical interview or history and should be taken accordingly. They represent his mother's subjective view of Mr. Sample.
The following is a CONFIDENTIAL REPORT meant for qualified Mental Health and AODA providers. While feedback of test findings to clients is highly encouraged and should be an integral part of therapy and treatment planning, clients should not be directly given copies of this report as they are likely to misunderstand report contents and their tentative nature due to their lack of Mental Health training.
CHILD SELF RATING
Mr. Sample completed the Personality for Youth to measure his self-perceived functioning.
VALIDITY
Mr. Sample appeared to have read test questions and did not answer in a random manner.
His consistency in response indicated deliberate response and good comprehension of test material.
Mild tendencies to exaggerate problems and pathology were seen in testing. Low self-image may lead to this pessimistic view of himself.
Mr. Sample did not minimize problems or presented himself in an overly positive light.
COGNITIVE FUNCTIONING
Mr. Sample reports severe difficulties with overall Cognitive functioning which is a source of dissatisfaction to him. Academic deficits probably hinder daily functioning and self-esteem. Memory, language, and mathematical skills were felt to be average. Moderate feelings of intellectual inadequacy exist as he feels unable to perform intellectually. This may produce a self-fulfilling prophecy. Many modifications of school programming are reported.
Mr. Sample characterizes his thoughts as being very consensual, typical and reality based. Goal directed thinking is described. An Average ability to Reality Test is reported. Mr. Sample feels an average ability to understand the world around him.
IMPULSE CONTROL
Extreme overall problems with patience, impulse control, frustration tolerance and ability to plan are reported. Mr. Sample feels almost totally unable to complete tasks and reach goals. Normal levels of autonomic arousal are said to exist. Moderate distractibility and over activity are described. Moderate difficulties with self control and responding to limits are reported. Severe Impulsivity is reported. Mr. Sample feels he has limited control over his own behavior.
ACTING OUT BEHAVIOR
Mr. Sample blatantly ignores social norms and manipulates others. Mr. Sample describes exceptional levels of breaking rules, testing limits and manipulating others. He reacts to his immediate needs and does not allow others to control him. Delinquent, acting out behavior is denied.
PHYSICAL FUNCTIONING
Physical health is said to be good. Psychosomatic like symptoms are not reported. An average concern with and focus on bodily functions exists.
EMOTIONAL FUNCTIONING
Moderate overall levels of negative emotions was described by Mr. Sample which may be clinically significant. Moderate worry, anxiety and fear are reported. Severely Depressed mood is described by Mr. Sample. Mr. Sample reports having exceptionally good sleep habits.
FAMILY AND SOCIAL FUNCTIONING
Mr. Sample reports mildly troubled relationship in his family. Reported level of Parent-Child conflict is average. Severe parental maladjustment is said to exist. Mild marital discord was described.
Mr. Sample reports feeling socially comfortable with others. He has mild tendencies to avoid being with others. He does not usually become fearful around others.
Extreme social skill deficits exist as he has severe difficulty interacting with others in a positive manner. Mr. Sample is very interpersonally disrespectful, intrusive, usually brash, and annoying which alienates others. Moderate peer conflicts are reported as Mr. Sample has trouble making and keeping friends. Peer relationships can be problematic for him. Severe problems with social status are reported. He feels extremely unpopular with peers.
